1px;">未达到国家法定退休年龄，户籍所在地、居住地（须持有当地居住证且在有效期内）在武汉市的中国公民及驻汉部队现役军人、现役武警，如已取得《中小学教师资格考试合格证明》(或《师范生教师职业能力证书》)且在有效期之内，可在我市申请认定高级中学、中等职业学校和中等职业学校实习指导教师资格。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">根据《教育部办公厅 中共中央台湾工作办公室秘书局 国务院港澳事务办公室秘书行政司关于港澳台居民在内地（大陆）申请中小学教师资格有关问题的通知》（教师厅〔2019〕1号）规定，在我市学习、工作和居住的港澳台居民，无犯罪记录，<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">持港澳台居民居住证可在居住地</strong>、持港澳居民来往内地通行证或5年有效期台湾居民来往大陆通行证等有效证件，可在<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">教师资格考试所在地</strong>申请认定中小学教师资格。申请认定教师资格的学历及其他条件、程序要求与内地（大陆）申请人相同。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;"><span style="-webkit-tap-highlight-color: transparent;outline: 0px;color: rgb(0, 122, 170);">三、相关条件标准</span></strong></span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">（一）认定权限。</strong>根据相关法律法规的规定，高级中学教师资格、中等职业学校教师资格、中等职业学校实习指导教师资格，由申请人户籍所在地（或居住地）县级人民政府教育行政部门审查后，报上一级教育行政部门认定。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">（二）学历标准。</strong>按照《中华人民共和国教师法》第十一条和省教师资格认定中心《关于我省教师资格认定学历问题的说明》（鄂教资〔2012〕4号)有关规定标准执行。即：</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">申请认定高级中学、中等职业学校教师资格应当具备高等师范院校本科或者其他大学本科毕业及其以上学历；</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">申请认定中等职业学校实习指导教师学历应为大学专科毕业及以上学历，同时还应具有相当于助理工程师或中级以上工人技术等级。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">（三）普通话要求。</strong>申请语文学科教师资格需达到二级甲等及以上标准。申请其他学科教师资格需达到二级乙等及以上标准。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">中小学高级教师、中等职业学校高级讲师、高级实验师、特级教师等均不可作为免试依据。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;">（四）体检标准。</strong>严格按照《湖北省申请认定教师资格人员体检标准及办法》（鄂教师〔2002〕3号）规定执行。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">体检时间自本公告发布之日起2个月内体检有效；申请人从武汉市教师资格认定体检医院名单（附件1）中，任选一家医院进行体检，使用湖北省教师资格申请人员体检表，所有体检项目必须完检；省人民医院为疑义问题终检医院。</span></section><section transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">（一）网上报名。</strong>申请人登录中国教师资格网（www.jszg.edu.cn），通过“网上办事”栏目下“教师资格认定”服务入口，点击“在线办理”进行账号注册和报名。注册方法可参考中国教师资格网主页面“咨询服务”栏下的“操作手册”。注册成功后，先完善个人信息，并在申请人员申请的认定机构网报时间段内登录报名。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">1.完善个人信息。</strong>对于注册后首次登录的或个人信息未完善的用户，须先完善个人身份信息。证件类型为身份证的，需要补充民族；证件类型为港澳台居民居住证、港澳居民来往内地通行证、五年内有效期台湾居民来往大陆通行证的，需要补充性别、出生日期、民族及港澳或台湾当地有效身份证号码；对于证件类型为港澳台居民居住证的，还需补充相应通行证号码。检查无误后，点击“提交”按钮，完成个人信息的完善和提交。</span></section><section 0px;">2.报名。</strong>网上报名时，请准确选择认定所在地类型：社会人员选择户籍地或居住证所在地；认定机构：申请高中阶段（高级中学、中等职业学校、中等职业学校实习指导）教师资格的，选择武汉市教育局为认定机构；现场确认点：社会人员选择户籍地、有效居住证所在地的区教育局为现场审核确认点。</span></section><section 1px;">申请人需按系统要求如实完整填写其他申请材料，核对所填报名信息，确认无误后点击“提交”按钮，上传报名信息，已审核的“教师资格认定申请信息”将不能修改，请申请人慎重填写申请信息。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 0px;">（二）现场确认。</strong>10月10日至10月15日组织现场确认，以各区现场确认点工作安排为准，详见《各区高中阶段现场确认点开设情况》（附件2）。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">申请人须在规定的时间，携带规定的材料到网报时所选的现场确认点参加现场审核确认，<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">未按要求在指定时间、地点参加现场确认的视为自动放弃申请认定。</strong></span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">（三）集中受理。</strong>市教育局受理现场确认点审核确认通过的申请材料，受理区教育局现场确认材料截止日期为10月29日。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;"><strong style="-webkit-tap-highlight-color: transparent;outline: 申请中等职业学校实习指导教师资格类别的申请人，除提供以上资料外，还需提供相当助理工程师及以上专业技术职务的职称证书或中级及以上工人技术等级的资格证书。<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">职称证书（或资格证书）须与申报学科相匹配。</strong></span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 1px;">（五）教师资格认定申请者在同一年度内只能申请认定一种教师资格，<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">请按规定时间、地点、要求进行网上申报和现场确认，因错过申报时间、选错认定机构或现场确认点、申报信息有误或提交材料不全等原因，未在规定时段内完成申报或补充材料的，认定机构将不再受理；</strong>网报时，申请人应仔细阅读网报说明，按要求实名注册认证，真实准确填报信息，<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">因错报、瞒报导致平台无法核验，所造成的后果由申请人承担。</strong></span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">（六）《思想品德鉴定表》改由《个人承诺书》替代后，社会人员的无犯罪记录证明，按属地管理原则，由各区在完成现场确认后对接辖区公安机关，发函进行背景核查。</span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">港澳台居民分别由香港特别行政区政府香港警务处、澳门特别行政区政府身份证明局和台湾地区有关部门开具。申请人需及时到所选择的认定机构开具《无犯罪记录证明函件》，申请人自行将函件交给香港、澳门及台湾相关部门，由其核实后将核查结果交给相关认定机构。核查结果将作为认定依据。<strong style="-webkit-tap-highlight-color: transparent;outline: 0px;">未能按时提供核查结果书面回函的，本次教师资格认定申请无效。</strong></span></section><section style="-webkit-tap-highlight-color: transparent;outline: 0px;text-indent: 2em;text-align: left;line-height: 2em;"><span style="font-size: 17px;letter-spacing: 1px;">申请人在申请认定过程中如有弄虚作假、骗取教师资格等失信行为的，或认定后经公安机关核查有犯罪记录的，将被纳入黑名单，依法受到失信联合惩戒、撤销或丧失教师资格的处罚，被撤销教师资格者自撤销之日起5年内不得重新取得教师资格；丧失教师资格者不得重新申请认定教师资格。</span></section><section style="-webkit-tap-highlight-color: 0px;">（七）申请教师资格认定有关表格使用要求。</strong>①《教师资格认定申请人信息核对表》是认定过程存档备查的依据，建议申请人下载该文档后使用电脑填写信息（不建议手填），使用A4纸打印、粘贴近期免冠正面1寸彩色白底证件照（应与网报上传相片同底版）、手写签名。②《湖北省教师资格申请人员体检表》使用1张A4纸正反打印，粘贴近期免冠正面1寸彩色白底证件照。③资料袋封面由申请人根据本人身份类型选择使用，建议使用电脑填写信息，A4纸打印。信息核对表、体检表及资料袋封面均可从附件3中下载使用。</span></section><section
